PaRC is happy to provide you with a copy of your medical record. To obtain a partial or full copy of it, download and complete a Medical Record Authorization Form. Fax your completed form to our Medical Records Department at the number below. A Medical Records Representative will contact you regarding payment for your record. You can expect to receive your medical record in approximately two weeks.
